The lengthening of the penis entails the release of the ligaments that attaches the penis to the public bone since about one third to one half of the entire penis length is inside the body and attached to the pubic bone. The two ligaments that are to be released are the fundiform ligament and the suspensory ligament.
Cutting the suspensory ligament is the most common method in penile length enhancement.
The release of the suspensory ligament lets the penis protrude in a straighter path outward, allowing for an increase in functional length. The results of this type of surgery are rather difficult to asses. This ligament keeps the penis pointing upward and, if cut, will no longer provide any support for the penis making the penis point down whenever it is erect.
Complications that may arise after this type of surgery include scarring, bleeding, infection, sloughing of the skin, and scrotalisation of the penis - a condition characterized by the penis appearing to rise from the scrotum.
